Thank you so much for giving such a structured and organised description of your problem. Such a description was required looking at the peculiar nature of the problem.
Few points:
1) How many Questions including module 1,2 and 5 are there in the complete course, also what is the total number of slides in project?
2) Can you check under Project : Advanced Interactions and let me know how many interactive objects are present on your course, , I need the number.
Do few tests :
a) Remove 3 Question slide from Module 1 and republish the course and verify the result on client end. Is the client able to proceed any further?
b) Other test is resequence your course, keep the Quiz Modules together, the Sequence 1,2,3,4,5,6 should have all the Content Slides followed by Quiz Modules
c) Upload the same original package on http://Cloud.Scorm.com and verify the results.
Please share the results.
PS: make sure you have updated your Captivate, it should be atleast 6.0.1.240
