Just a question: did you by chance use two monitors recently? It happens that CP thinks the other monitor is still present and dialog box is on that monitor.
Second possibility, I experienced it more with CP7. if you have several windows open (with other applications, documents), the dialog box sometimes is hidden behind those windows. Try to check that.
