Player bar not appearing on publish

I built a few sample screens for an Adobe Captivate 6 project I am working on. The custom player bar worked until I added some multiple choice questions. And after I set to publish to HTML5.

I cannot get the player bar to appear again even after de-selecting publish to HTML5. Is there some conflict with quiz questions? The player bar is not appearing on any screen, even non-interactive screens. Is there some obvious reason this is not working? When I open the skin editor, the player bar options are all grayed out.
